Responsibilities of the Student
Responsibilities of Community Health Services (CHS)
When accepting students on a placement, CHS are responsible for ensuring:
Supervisor holds qualifications and experience in the relevant discipline
Supervisor understands the requirements of supervising students
Agency Student Placement Policy and Procedures are followed
Grievance/Conflict resolution procedures clearly stated and understood
Supervisors liaise with relevant personnel at educational institution as appropriate
Student registration forms are completed and forwarded to Human Resources/ Student Placement Coordinator
Timely and appropriate orientation to agency, discipline area and team
Development of an Agreed Learning Contract following consultation with, student, supervisor and educational institution, covering:
Responsibilities of both the student and supervisors are within their relevant scope of practice
Adequate supervision and support to students
Where possible any staff member taking on the role of student supervisor attends a recognised supervisors training course at a relevant educational institution
Responsibilities of Educational Institutions
When placing students on clinical placements, educational institutions are responsible for ensuring:
Student meets the minimum theoretical component to support their clinical or other objectives of the placement
Student skill level is at the necessary standard for the proposed placement.
Agency staff involved in the supervision of a student considered to be challenging are given information regarding their current level of competence and areas requiring strengthening (within the boundaries of student confidentiality and privacy)
Adequate information is given to students regarding the importance of confidentiality prior to placement
Sufficient training material and information is provided to supervision staff
Appropriate liaison with relevant personnel at CHS
Police checks are completed
Working with Children Check is completed if appropriate
Participation in the development of an Agreed Learning Contract for each student
Whenever possible to participate in the community health orientation programme
Adequate orientation is given by CHS
Opportunities for exposure to other disciplines are monitored and facilitated where possible,
Allocate placement supervisor from educational institution to support student while on placement
Provide regular contact/visits to students during placement, at a minimum, this should include phone contact at the commencement, midway and end of the placement
CHS are informed of any contact with student during placement
Lines of communication between all parties are clear and understood
Responsibilities of the Student
Students undertaking a placement are responsible for ensuring:
They are aware of the details of their educational institution coordinator and contact person at the CHS

They act in accordance with the educational institution's policies and practices and in doing so, maintain effective relationships with the CHS & institution whom they represent
Their professional code of conduct is maintained at all times
Privacy and Confidentiality agreements are signed and adhered to
The policies, procedures and protocols of CHS are followed
Grievance/conflict resolution procedures are understood
Student ID and/or CHS ID is carried on them at all times
CHS is provided with copy of Police Check &/or Working with Children Check forms
The Agreed Learning Contract reflects placement objectives and includes:
Specific project student to undertake
Placement tasks
Role of student
Goals and objectives of placement
Specific competencies to be developed
Expected outcomes of placement
Evaluation process for student, from the perspective of student, health service and teaching institution
Attendance at orientation and becoming familiar with agency and any orientation material provided
They notify their supervisor if training is insufficient to meet the demands of their tasks
Their understanding of their capacity and scope of practice
Commitment to and attendance at arranged supervision and meeting times
Lines of communication between themselves and the CHS and academic institutions are clear and understood
They report to academic institution &/or CHS where there are insufficient resources
