
Designing a tool to assess the suitability of supports and guide service design for HACC PYP clients
Within the context of the broader HACC PYP review, this project’s original aim and consultation scope was revised. The project work undertaken to date has included seeking early sector guidance to develop two draft assessment modules with content identifying the needs of children and young people, and younger adults as HACC PYP consumers. These two draft modules incorporate broad sector feedback and suggestions around preferred assessment template formatting. The information collated throughout this project will inform the future HACC PYP review work to be undertaken in line with the National Cabinet agreement to pursue the Foundational Supports for people with disability outside of the NDIS.

MARAMIS gap analysis and implementation in community health
After five of its member community health services indicated interest in support to increase the sector's confidence in the implementation of MARAMIS, the VHA successfully obtained a MARAMIS Sector Capacity Building Grant from Family Safety Victoria (FSV) in November 2021.

GPs in community health services learning network
The VHA has been funded to coordinate a learning network that supports community health services to improve the sustainability of GP-led services. Targeted learning opportunities will focus on leveraging the MBS and delivering integrated and multidisciplinary GP-led models of care.

Identifying the current state and contemporary training needs of the CHS HACC PYP workforce
The Department of Health has commissioned the VHA to undertake a project in which we will work with community health providers to support quality, safe, and contemporary practice by identifying the current state and contemporary training needs of the community health HACC PYP workforce (training matrix).

Supporting services to prepare for the aged care Support at Home reforms
In response to significant reforms and the impact of the sector's response to the COVID-19 pandemic, the Victorian Department of Health wants to support public health services and community health services delivering 'Support at Home' services.

Public health system opportunities for persistent pain
This project will unpack current public health system capacity, capability, challenges, and opportunities relevant to persistent pain and (more specifically) TAC clients; and design a proposed operational model that is compatible with best practice clinical evidence, and enhances client and provider experience, as well as value-based outcomes.

The community health response to the COVID-19 pandemic
In October 2020, the Department of Health commissioned the VHA to document the community health sector's pandemic response, the lessons learned, and the service adaptations and innovations that offered sustainable benefits to Victoria's healthcare system.

Supporting the High Risk Accommodation Response program
We delivered multiple activities relating to the 2020-21 High Risk Accommodation Response (HRAR) program – a program that provided outreach support to people living in settings where transmission of and vulnerability to COVID-19 were considered to be relatively high.

Resilient Communities model of care proposal
In March 2022 the VHA presented a proposed model of care for the continuation of the community health-delivered High Risk Accommodation Response (HRAR) program beyond the 2020 to 2022 COVID-19 pandemic period.

Recognising and responding to elder abuse in residential aged care
The 'Recognising and responding to elder abuse in residential aged care' resources were developed with funding from the Victorian Government and support from a reference group comprising practitioner and consumer representatives and subject matter experts.

The VHA partnered with Deakin University to undertake a research study titled 'Psychosocial impact of COVID-19 on community health service staff.

Vaccine Ambassador Program
The COVID-19 Vaccine Ambassador Program (VAP) supported the coronavirus (COVID-19) vaccination rollout in Victoria by building trust and confidence in the program and addressing misinformation in the community.

Supporting the implementation of the revised Community Health – Health Promotion guidelines
The VHA is supporting the Department of Health by developing a measures suite aligned to the department's revised CHHP guidelines that will operate as transitional from October 2022 until September 2023.
